6/25/2023 EnchantaVaughn Approved Behavior: Client verbalizes positive feelings about herself but recognizes areas she needs to work on. Client smiled and hugged QMHP. Client was observed in her seat and raising her hand during the activity. Client presented a good attitude throughout the session. Client appeared to be in a good space and dealing with her personal challenges one day at a time, as reflected in her growth and new habits. Client talked a lot about the good things that happened today and less about things that didnât matter. Behavior: Client displayed good participation and communication throughout the session. Client seemed to be in a calm mood, as evidenced by her positive expressions and relaxed demeanor. Client displayed appropriate social behaviors in the home. Client was seen displaying positive social behaviors, working well with others and actively engaged without causing disruptions. Client appeared to be in a content mood, as evidenced by her straight facial expressions and positive body gestures. Client was praised for having a wonderful end to her day and encouraged to keep up the good work. Behavior: Client seemed to be handling herself well in the home and following the rules and adapting to the in-home schedule and routine without feeling or needing to stop the session, decreasing all attention seeking behaviors in the home. Client states that she is going to have a good day and utilize her relaxation techniques when she needs recoup. Client expressed a positive attitude toward authority, as evidenced by her saying please and thank you. Client displays an increased pattern of independence through engaging in therapeutic activities Intervention: QMHP facilitated activity to allow client insight into several conflict resolution and problem-solving skills to utilize at home. QMHP encouraged the client to role play appropriate and inappropriate conflict resolution skills in efforts to highlight and demonstrate several appropriate options the client may utilize when she feels challenged or the need for impulsive disruptive actions. QMHP provided strategies such as positive reframing. QMHP assisted client with practicing reframing negative thoughts with more positive thoughts in efforts to reduce impulsive reactions to situations. Intervention: Client was able to reduce physical overactivity and become more attentive in session. QMHP discussed with the family one goal they believe they need to work on individually and as a family. QMHP had each member write down goals and discussed through brainstorming and creating a goals chart on how to achieve each one. QMHP explored positive behavior options she was exhibiting with client to support her willingness to continue to make good decisions and display appropriate behaviors in the home. Intervention: QMHP explored positive behavior options with client to support her willingness to make good decisions and have positive behaviors at home, in school, and in the community. QMHP commended client on her positive behaviors. QMHP explained and emphasized to client the importance of having a level grip on utilizing her coping skills to avoid mood swings and to promote positive thinking and self-motivation. QMHP assisted client with managing her anger and frustration by reviewing and role-playing effective coping skills with client. Response: Client demonstrates problem solving by staying calm and using her senses. Client acknowledges expectations and reviews goals and objectives with counselor and parent at the beginning of each session. Client displays good social behavior, as seen sharing and taking turns. Client implemented the positive reinforcement provided by her counselor and was able to stay motivated and enjoy her day. Client exhibited good manners after earning incentive for her hard work. Client did a good job ignoring distractions and maintaining appropriate behavior in the home. Response: Client focused on things she is good at and loves to do at home in order to stay motivated in the school/community setting and make something out of herself. Client administered positive behavioral expectations, reducing ADHD. As the client has made more of her own decisions, she reported increased satisfaction and function. Client responded well to feedback, interacting well with her peers. Client agreed to behave and communicated a positive attitude when reviewing problem solving and self-control strategies. Response: Client displayed good participation in various settings and was responsive to all instructions. Client understands that she is expected to act civilized and treat others with respect. Client responds well to praise, support, and positive reinforcement provided by QMHP and is seen increasing self-esteem and confidence. Client said she was going to practice coping strategies and relaxation techniques in all aspects of her daily living routine in hopes that she will be a better daughter and friend. Client was able to cooperate and comply to rules and regulations due to positive peer influence.
